What does Care Coordination mean? 

Care Coordination is the centerpiece of CCBHC programing.  The Agency for Healthcare Research and Quality (2014) defines care coordination as, “deliberately organizing patient care activities and sharing information among all of the participants concerned with a patient’s care to achieve safer and more effective care. This means the patient’s needs and preferences are known ahead of time and communicated at the right time to the right people, and this information is used to provide safe, appropriate, and effective care to the patient.”

 

All clients in a CCBHC environment will get care coordination.  This is not a billable service, but a required activity.  Think of it as “closing the loop.”  This activity is the follow up with other providers and agencies to make sure care is delivered and results are communicated between PVC and other services.  This may be done by the case manager or by a specialized care coordinator.

All families with children with autism and other special needs can enjoy the time-honored tradition of a visit with Santa, in a more subdued and calm environment.. Sensory-friendly visits with Santa are free and keepsake photo packages will be available for purchase.
